APCC Monthly Measures

These are the current measures for the APCC Program.

Diabetes

  • The total number of patients on the diabetes register
  • The percentage of patients with diabetes whose last recorded HbA1c within the previous 12 months was:
    • Less than or equal to 7.0%
    • Greater than 7.0% but less than or equal to 8.0%
    • Greater than 8.0% but less than 10.0%
    • Greater than or equal to 10.0%
    • Not recorded
  • percentage of patients with diabetes whose last measured total cholesterol was less than 4 mmol/l within the previous 12 months
  • percentage of patients with diabetes whose last recorded Blood Pressure (BP) reading was less than or equal to 130/80 mmHg within the previous 12 months
  • The Percentage of patients with Diabetes that have had diabetes Service Incentive Payments (SIPs) claimed for them within the last 12 months

Coronary Heart Disease (CHD)

  • total number of patients on the CHD register
  • percentage of patients with CHD on aspirin
  • percentage of patients with CHD on a statin
  • percentage of patients who have had a Myocardial Infarction (MI) in past 12 months who are on a beta blocker
  • percentage of patients with CHD whose last recorded Blood Pressure (BP) within the last 12 months was less than 140/90 mmHg

Access and Care Redesign

  • patient survey
  • unmet demand
  • The Number of days until the GP 3rd Available Appointment
  • The Number of days until the Practice Nurse 3rd Available Appointment
    (Where there is not a Practice Nurse or the Practice Nurse does not have appointments this measure is not required)
