Local Collaboratives

The Local Collaborative Program is designed to allow greater learning flexibility for practices to participate in the Australian Primary Care Collaboratives (APCC) Program. In Phase 1 the Local Collaboratives program had more than 84 practices from 12 Divisions/Consortia running Orientation days and Workshops in their local area.

The Local Collaboratives program uses the same methodology as the core Collaborative program with Orientation Days, Learning Workshops and Activity Periods and participating practices have access to the same comprehensive suite of resources used in the core program, including the APCC Handbook, practice presentations, and change ideas to guide them through the process of quality improvement to their systems. Practices are also required to submit monthly data in line with measures for each topic.

The flexibility of the program allows sharing of information while building on the successful outcomes achieved by previous participants in the APCC program. The Local Collaboratives will be run by Divisions in a similar format to the Core program in the topic areas of Access, Diabetes and Coronary Heart Disease (CHD).
