New APCC Program Measures

The Improvement Foundation Australia is pleased to announce we have confirmed new measures for the APCC Program.

Why is the Program introducing new measures?
The new measures reflect the latest evidence as to the targets that produce better outcomes for patients. They also cover a wider area of diabetes and CHD targets than the current measures.

How were the new measures selected?
The new measures were selected by expert reference panels that included GPs, practice nurses, specialists, and representatives from the RACGP, Heart Foundation and Diabetes Australia.  

How will the new measures benefit practices and patients?
Measuring enables practices to have access to clinical data that can guide  improvements. Practice’s use their clinical data to improve their systems and patients will benefit from more systematic care.

How will the new measures benefit Divisions?
Divisions will get great value from using the measures to influence population health strategies. The new measures will also support quality improvement work being done at Divisions.
