How to's & User Guides for Collecting Measures
To help you collect the baseline and regularly monthly measures we have created the following "How to'" guides . Where possible we have worked closely with the software providers to produce this information, but cannot guarantee "how to" guides for the many variations of medical software available.
If you are require further assistance with collecting the measures please contact your Collaboratives Program Manager at your Division or the APCC Team.
Data is submitted on the first Wednesday of each month.
